A patient at the end of a course of physio is often ready for more than the clinic can give them. The rehab has done its job, the injury has settled, and they want to get fitter and stronger without undoing the work. A physio referral partnership with TriageFit gives that patient a coached training programme built around the injury you treated, at a discount only your link carries, and gives your clinicians a view of their progress if the patient chooses to share it.
What the partnership gives your clinic
Registration gives the practice its own portal, and everything the partnership runs on lives there.
- A partner link, one press away in the portal. Every patient who joins through it gets 10% off every payment for as long as they subscribe, shown on each receipt as "Clinic partner discount".
- A printable one-pager, written to the patient, with the link, a code to scan and the sharing choice set out in the same words they read again at checkout.
- Clinician seats. Each clinician you invite holds a healthcare provider seat, which opens the Patients tab and a printable progress report for every patient who shares.
- A cohort report, a printable document of group figures across the patients who share with you.
Registering the clinic
Registration takes a few minutes on the clinic registration page. It asks for the clinic's name, your name and a contact email. The clinic's website, your role and a rough count of clinicians are optional, though the website makes verification quicker. If an affiliate introduced you, name them in the "Who referred you" box so their referral counts, then tick to confirm you are authorised to register the clinic and accept the clinic partner terms.
Submitting creates your organisation, your portal and your partner link straight away, so you can invite your clinicians and print the one-pager the same day. The link and the discount work from that moment. We then check by hand that the clinic is real and that the person who registered it speaks for it, and email the contact with the outcome. Sharing opens once the clinic is verified, so a patient is only ever asked to share with a practice we have checked.
Referring a patient
When a patient is nearing discharge, hand them the one-pager or send the link in a message. They sign up through it, pay for their membership with the discount applied and join your clinic's organisation as a patient. Their intake opens as soon as they have paid.
The intake takes about thirty minutes and can be saved partway. It asks about their goals, the training they have done before, injuries and restrictions, how their week runs, how long they have for a session and the equipment they can reach. The injury you treated goes in there, and the programme is built around it from the first session, with the exercises that aggravate it left out. A few answers in the health screen, such as chest pain during activity, leave the patient without a programme until a GP has told them exercise is safe and they have recorded that in the app.
What your patient gets from TriageFit
Your patient trains in phases our coaches designed, each four to six weeks long and cycling for as long as they train, with every slot filled from a library of 382 exercises to suit their equipment, their injuries and their level. The effort each session asks for adjusts to how they are recovering. Their first check-in comes round at the end of the first week, on a day they choose. A coaching message about the week arrives within seconds of sending it, the next week is rebuilt around it, and a real coach reads the same check-in.
Between check-ins the AI coach answers at any hour. If something starts to hurt, it proposes a swap that works around the problem and tells the patient when a physio should see it, and nothing it suggests changes the plan until the patient confirms it. The sharing choice, in the patient's words
Once your clinic is verified, a patient who joined through your link is asked in plain words, at checkout or later in their settings, whether to share their progress with your clinicians. They can decline, their membership and coaching are the same whichever they choose, and they can change their mind at any time in their settings.
While sharing is on, your clinicians see the patient's name, email address, date of birth and gender, the programme week they are on, when they last checked in, how closely they are following the plan, their health scores and which way they are heading, blood work from the last year, their medications and injuries, the energy, stress and sleep from their last two weeks of check-ins, and how long it has been since they last used the app. A patient who has stopped using it for a while is marked on the list as disengaged, so a quiet patient stands out.
Their journal, their messages with their coach and their day-to-day training and food logs stay between them and their coach. Adherence reaches you as one figure, never as the sessions themselves. A patient who turns sharing off leaves your clinicians' patient list straight away and drops out of your cohort report with it.
What your clinicians see, and who can see it
Only people your clinic's admin invites into a clinician seat can open a patient's summary, and the terms ask you to give those seats only to clinicians involved in your patients' care and to remove a seat when someone leaves. Every read of a summary or a report is logged with who read it and when.
The Patients tab lists everyone sharing, with each patient's programme week, last check-in, adherence figure and the direction their scores are moving, and a printable report for each patient. The cohort report covers the patients who share as a group, and any group smaller than five people is withheld so that no figure can point to one patient. For anything a clinician reads under sharing, the clinic is a controller in its own right under data protection law, and TriageMethod Ltd stays the controller of the service and of each patient's membership.
What it costs and what it can earn
The partnership costs the clinic nothing. There is no fee to register, your clinicians' seats are part of the partnership, and each patient pays for their own membership at the listed €50 / 4 weeks or €500/year, less the 10% discount, for as long as they stay.
Where your professional rules allow a commission on a referral, you can switch on 5% of every payment your referred patients make, for as long as they stay. It is off until you turn it on in your clinic settings, the patient pays the same either way, and only you can say whether your regulator permits it. On the four-weekly plan each payment after the discount earns the clinic €2.25, or €29.25 over a year, so 10 patients who stay the year come to €292.50. Funding places for patients
Some clinics would rather pay for the membership themselves, for a rehab class or a group finishing treatment together. Seat packs are sold at the prices companies pay, €500 a seat for 10 to 24 seats, €450 a seat for 25 to 49 seats, €415 a seat for 50 to 99 seats and €375 for 100 or more, each seat covering a year. Packs of 10 to 50 seats can be bought online under the corporate terms, and each seat reaches a patient by invitation. A funded seat pays for the membership and changes nothing about sharing, which only ever comes from the patient's own choice.
Ending the partnership
Either side can end the partnership by email at any time. Sharing ends, the link stops taking new patients, and the clinician seats and the portal close. Every patient you referred keeps their membership and their history.
The paperwork for a practice manager
If a practice manager or data protection lead wants the details in writing before anyone registers, they are all published.
