Coached exercise for your patients after physio discharge

TriageFit gives a patient leaving your care a coached programme for exercise after physio discharge, built around the injury you treated and adjusted every week from their check-in. Joining through your clinic's partner link takes 10% off every payment, and a patient who chooses to share lets your clinicians follow how they are getting on.

The discharge appointment is a good day for a patient and an open question for the clinic. The injury has settled and they are ready to do more, but the regular sessions are ending, and what they train on next is up to them. For a patient who wants to get back to the gym, run again or get properly strong, exercise after physio discharge goes better with a plan that knows about the injury. A partnership with TriageFit gives you one to hand them, a coached programme built around what you treated, at a price only your clinic's link carries.

Where the clinic's part ends

By discharge the clinic has done what it is there for. The patient has been assessed, treated and brought back to function, and the home exercises on their sheet cover the maintenance. A patient who wants more than maintenance is asking for something else, a training programme that moves forward week by week, changes when a week goes badly and keeps the old injury in view the whole time. That is a service in its own right, and it is the one TriageFit provides.

What the patient trains on

The patient starts with an intake of about thirty minutes, which they can save and come back to. It asks what they want to get back to, the training they have done, their injuries and restrictions, the shape of their week, how long they have for a session and the equipment they can use. The injury you treated goes into those answers, so the programme works around it from the first session and leaves out the exercises that aggravate it. Some answers in the health screen, chest pain during activity among them, stop a programme being built until the patient has the all-clear from their GP and has confirmed it on their plan page.

Training then runs in phases our coaches designed, each four to six weeks long, with every session drawn from a library of 382 exercises matched to the patient's equipment, injuries and level. How hard each session asks them to work follows how they are recovering, so a poor week of sleep or a sore few days is taken into account.

The first weeks after discharge

Take a patient signed off after an ankle sprain who wants to be running again by the summer. They join through your link on the day of their last appointment and do the intake that evening, with the ankle in their answers, and their first week is built around it. On the check-in day they chose, they report how the sessions went, a coaching message about the week arrives within seconds, the next week is rebuilt from it, and a real coach reads the same check-in.

When the ankle complains after a longer run, they tell the AI coach, which answers at any hour. It proposes a swap that works around the problem and tells them when a physio should see it, and their plan stays as it was until they confirm the change.

Handing over the referral

Registering the clinic takes a few minutes and gives you a portal with your partner link and a one-pager to print. Hand the one-pager over with the discharge sheet, or send the link in a message after the appointment. A patient who joins through it pays 10% less on every payment, on either plan, and their receipt shows the saving as a clinic partner discount.

The link works from the moment you register. Once we have checked the clinic by hand, each patient who joins through it is asked, in plain words, whether to share their progress with your clinicians, so nobody is ever asked to share with a practice we have not verified.

What you can follow if they share

Sharing is the patient's choice, and their coaching is the same without it. If they say yes, your clinicians can see their programme week, their last check-in, how closely they are following the plan as a single figure, their health scores and which way they are moving, and their injuries, along with the rest of the summary set out in how the partnership works. A patient who has gone quiet for a while is flagged on the list, and each one has a printable report.

Their journal, their messages with their coach and their day-to-day logs stay private, and a patient who turns sharing off drops off the list straight away. Home exercise programme adherence looks at what keeps a patient training once the appointments stop.

What it costs the clinic

The partnership costs the clinic nothing to join or to run. There is no registration fee, your clinicians' seats come with it, and the patient pays for their own membership, less the discount, and can pause or cancel from inside the app. Where your professional rules allow a commission on a referral, the clinic can also switch on 5% of each payment those patients make, and extra revenue for physio clinics works through the figures.

How to register

The clinic partnership page has the full detail, and you can register the clinic from there. The partner link is ready as soon as you submit, the clinic one-sheet answers a practice manager's first questions, and the one-pager can go out with your next discharge.

Questions people ask

When is a patient ready to be referred?
That is your judgement as their clinician. The partnership suits a patient you are discharging who wants to keep training. The intake asks about their injuries and restrictions, and some answers in its health screen, such as chest pain during activity, mean the programme waits for their doctor's go-ahead, which they record on their plan page.
What happens if the injury flares up again?
The patient tells the AI coach, which proposes a swap that works around the problem and tells them when a physio should see it. Nothing in their plan changes until they confirm it, and a real coach reads their weekly check-in, where a pain that keeps coming back shows up.
Does a patient have to share their progress with us?
No. They are asked in plain words, at checkout or later in their settings, and their membership and coaching are the same whichever they choose. They can turn sharing off at any time. A patient who declines still keeps the discount from your link for as long as they subscribe.
Can we hand out the link before the clinic is verified?
Yes. Registering creates the partner link, the portal and the one-pager at once, and the discount applies from that moment. The sharing question only reaches patients once we have checked the clinic, so a patient is never asked to share with a practice we have not verified.
